PSL in Your Office

You can make the option to register as an organ and tissue donor available to your patients

How the Program Works

While more than 1 million people benefit from organ and tissue transplants each year, we are still tragically missing thousands of donation opportunities because potential donors have not been registered.

This video and the links below will give you the tools you need to provide your patients the opportunity to join the donor registry.

10,000-12,000 people die every year for lack of donated organs.

General Procedures

  • When a patient checks at the front desk, the PSL form should be given to them for completion.
  • Every form should be collected by the front office (even if it’s blank) and saved until the end of the day.
  • All forms should be mailed to CORE as part of the end of day procedures in a postage paid envelope. The forms can be mailed daily or weekly depending on the volume of the practice. It does not matter if the forms are blank; this is all valuable information for our research team.
  • We can help you devise a protocol to avoid duplicate questionnaires by determining if a patient has already been given a form at a previous visit.
  • These processes can be modified according to the needs of each office and tailored to make the modification as seamless as possible.

UPMC Procedures

  • When a patient checks in with the EPIC System, the PSL form is automatically generated with their name and address filled in. Every form should be collected by the front office (even if it’s blank) and saved in a pile until the end of the day.
  • All forms should be mailed to CORE as part of the end of day procedures in a postage paid envelope. The forms can be mailed daily or weekly depending on the volume of the practice. It does not matter that the forms are blank; this is all valuable information for our research team.
  • These processes can be modified according to the needs of each office and tailored to make the modification as seamless as possible.
  • The system does not store any information about your patient’s decision. It only stores the DATE on which they were given the opportunity to designate as a donor. This way, the system will only have to ask once a year.
  • This approach can be used by any provider or health system that uses the Cadence component of EPIC.
